What. PRD revised: human-review goal row (workload metric, target from M1), non-goals split into permanent vs deferred (dev-only inspection UI carved in), I4 changed-source re-ingest requirement, M1 extended with review-driver analysis, and three new requirement groups (R human review and auditability, O/C observability and cost, B/X backup and security). Decisions log extracted from the PRD into docs/decisions.md (ADR-lite, D1-D9 migrated, D10-D14 added).

Why. Review of the draft PRD with Agus, 2026-09-01. Rulings D10 (human-capability ceiling, precision over queue-minimization), D11 (re-ingest semantics), D12 (action log), D13 (LLM interface), D14 (no erasure, accepted risk) carry the reasoning.
